The Commonwealth of Pennsylvania requires any employee or volunteer above the age of 13 who works with children to complete three background checks. The purpose of these checks is to screen for those with a history of perpetrating child abuse. These background checks are mandatory, and must be renewed every 3 years. We will keep this information private and secure in the church office.

 

I. PA Child Abuse History Clearance

  1. Go to: https://www.compass.state.pa.us/cwis/public/home
  2. Create an individual account, known as a “Keystone Key”
  3. You will need to give personal information, including residential addresses for the last 7 years. Helpful hint: if you use Amazon, you may have stored past addresses there
  4. When the check is complete, you will receive an email to log into your account and access the results
  5. Print these results and return to the Director of Lifespan Faith Development or Office Administrator

 

II. PA Access To Criminal History (PATCH)

  1. Go to: https://epatch.state.pa.us/
  2. Select “New Record”
  3. Certify that you are requesting the check for a volunteer position (fee is waived)
  4. When the check is complete, you will receive an email
  5. Print these results and return to the Director of Lifespan Faith Development or Office Administrator

 

III. FBI Background Check

NOTE: If you have lived in Pennsylvania for at least the past ten consecutive years, you may sign an affidavit provided by UUCV rather than completing this check. See the Director of Lifespan Faith Development or Office Administrator to complete this. Otherwise, you must request an appointment to be fingerprinted.
